The NHS Digital Weight Management Programme could help you if you are living with obesity in Southall or Hillingdon and also have diabetes, high blood pressure, or both.

If this is you, you could benefit from this 12-week programme. It’s free and easily available via a smartphone, tablet, or computer.

How can I join the NHS Digital Weight Management Programme?

  • You must be 18 or over.
  • You must have a BMI greater than 30. The BMI threshold will be lowered to 27.5 for people from black, Asian, and ethnic minority backgrounds, as we know people from these ethnic backgrounds are at an increased risk of conditions such as Type 2 diabetes at a lower BMI.
  • You must have diabetes, high blood pressure, or both.
  • You must have a smartphone, tablet, or computer with internet access.

Referral

After you have consented to the programme with our Pharmacist, you will receive a text message within two working days from ‘NHS WMP’.

The text message contains a link to a website where you can learn more and start your journey.

Your Plan

Choose the plan you think will work best for you.

If you do not choose your plan after six weeks, your referral is sent back to the referring healthcare professional.

Your Journey

You will then start your 12-week weight management plan!

You will be supported along the way.
